Describe the approach you currently take with patients and their families when discussing the need to shift from curative/active treatment to palliative treatment. How might you incorporate the PPS into this approach?

What are some of the challenges your team has had helping patients transition from active to palliative treatment? Would using a common assessment tool such as the PPS aid in overcoming some of the barriers? If so, how?
